The purpose of the Enhancing Military-Civilian Medical Synergies study (2016) was to assess the state of Army medical practice in civilian facilities and identify opportunities for greater synergies. The study introduces the fact that there is a distinction between medical care demands in combat settings and the needs Army medical treatment facilities (MTF) located within the United States, which justifies the need for military medical professionals to operate within civilian hospitals, trauma centers, operating rooms and intensive care facilities.Due to the difference in services provided domestically, there are many benefits to MTF-based care providers providing direct care within civilian facilities. One type of agreement allows for MTF-based care providers, mostly physicians (typically surgeons), to provide direct care to Military Health System beneficiaries at civilian hospitals; there are similar resource-sharing agreements with U.S. Department of Veterans Affairs (VA) medical centers; and, at some locations, Army MTFs share medical resources with other services in multiservice markets. These arrangements are largely intended to improve the timing, quality, and efficiency of care for beneficiaries, but they also help military medical professionals maintain clinical proficiency. Other agreements enable military medical personnel to provide care to civilian patients, in a training context.The study found that of 28 Army MTFs, 13 have agreements for military medical personnel to provide care in VA or other civilian facilities. The most frequent reason for these agreements are to serve beneficiaries and meet routine proficiency maintenance needs. Often these agreements allow medical personnel to practice types of care not offered at a MTF as well as access training for medical readiness needs and deployment-related needs. Common themes from both military and civilian stakeholders are that these agreements are mutually beneficial. Benefits include access to quality care for patients; opportunities for Army medical practitioners to be exposed to industry best practices in civilian facilities; access to medical technologies that might not be available in MTFs but used more commonly in civilian facilities. Military personnel also cite good community relations as a benefit.Many factors contribute to the MTF’s decision about how and where they can best meet their various mission related requirements. In situations where MTFs have made agreements with civilian facilities, each MTF develops its own business plan based off of an assessment of the local profile and alignment with the needs of the MTF personnel, as well as cost considerations. Considerations for selecting a facility may include: patient mix, infrastructure, service, equipment and cost.A conclusion of the study recommended further enhancement of military-civilian medical synergies including identifying potential untapped opportunities for MTFs to utilize external practice, analysis of the cost and efficiency associated with further agreements, and if warranted bases on the analysis, encouraging the expansion of these agreements to include a wider range of Army medical professionals and medical teams. Home improvement retail giant Lowe’s has announced it will close 31 locations — mostly under the Rona banner — in Canada.In a release, the company said the stores being closed are under-performing.“While decisions that impact our associates are never easy, the store closures are a necessary step in our strategic reassessment as we focus on building a stronger business,” Marvin R. Ellison, Lowe’s president and CEO, said in a statement.“We believe our people are the foundation of our business and essential to our future growth, and we are making every effort to transition impacted associates to nearby Lowe’s stores.”Three stores in the GTA have been marked for closure: two Rona locations in Mississauga and the Lowe’s at Centrepoint Mall.In total, there are 31 sites that are closing: two regional support centers, a truss plant, a block plant and 27 stores. VICTORIA — Ride-hailing companies could begin operations in British Columbia by next fall under legislation introduced today.Transportation Minister Claire Trevena says the government’s bill strikes a balance between meeting consumer demand and protecting public safety.Among other things, the bill would give the Passenger Transportation Board the power to accept applications and set terms and conditions for licences covering taxis and ride-hailing services like Uber and Lyft.It would also set rates and determine the number and coverage areas of the services.The legislation would see the creation of a legislative committee to review and make changes to the system as well.Green Leader Andrew Weaver says the NDP’s bill is a step forward, but he questions why the government has chosen to require driver training and class five licences rather than class four, which he says have been used successfully in Quebec.Trevena says the experience of other jurisdictions has been used to develop British Columbia’s legislation, which is aimed at preventing gridlock, maintaining ridership on public transit, and reducing accidents caused by unsafe or inexperienced drivers.Vancouver is one of the few major cities in Canada that does not permit ride-hailing.The New Democrats, Liberals and Greens promised to bringing in ride-hailing during the 2017 provincial election campaign.An all-party committee of the legislature made 32 recommendations last February to help pave the way for ride-hailing.The report highlighted five key areas that needed to be considered when establishing regulations for the industry including pricing, insurance, licensing, and public safety. The committee also recommended updating legislation that regulates the taxi industry to “allow for equitable and fair competition.” The Canadian Press
Kolkata: Two mountaineers from the state on Kanchenjunga expedition — Kuntal Karar and Biplab Baidya — are believed to be dead as the rescue operation to bring them back to the base camp proved futile.The Sherpa team, which was on the rescue job of the two injured Indian climbers Biplab and Kuntal, has returned to Camp-4 of Kanchenjunga, as reported from the base camp. The herculean rescue operation, performed by Sherpa Nirmal Purja and his team on Wednesday, yielded no positive outcome. Also Read – Bengal family worships Muslim girl as Goddess Durga in Kumari PujaUnesh Zirpe, the leader of the expedition team Giripremi from Base Camp, said: “There is almost no hope that any further rescue operation is possible to bring Biplab and Kuntal back to Camp-4 at this stage.” The handling agency has also made it clear that there is almost no possibility that any further rescue effort can save these two lives. Ramesh Ray, another climber from the same team, was rescued by the Sherpas and brought to Camp-4 around 10 pm on Wednesday. His health is on the verge of critical deterioration and he was suffering from altitude sickness leading to HAPE (high altitude pulmonary edema) and snow blindness. Climber Rudraprasad Halder, who was at Camp-4 on Thursday morning, was also suffering from frostbite. Later in the day, both of them were evacuated by rescue helicopter and brought at the base camp. Biplab, Kuntal, Ramesh, Rudraprasad and Sk Sahabuddin were part of a five-member team from Bengal, who had embarked on the Kanchenjunga expedition. Four of them — Rudraprasad Halder, Ramesh Ray, Sk Sahabuddin and Biplab Baidya — had reportedly reached the summit point on Wednesday morning. However, Biplab and Kuntal fell sick while descending and sought help from an altitude above 8000m. It was believed that Kuntal was suffering from HAPE. One team member (possibly Rudraprasad) sent an SOS to their family around 3:12 pm on Wednesday via GPS device asking for urgent help. The device read an elevation of 8271m. Nirmal Purja with two other Sherpas — Gesman Tamang and Mingma David Sherpa — responded promptly to the request assuring a rescue operation from an altitude of 8200m. The sherpas were on their way down from the summit of Kanchenjunga.
The weakest division of the American League so far this season is the one that has been the most dominant in recent years: the AL East — featuring the Yankees and Red Sox, the two winningest AL teams since the league expanded to three divisions in 1995.Baltimore is leading the division with a puny 16-14 record, the worst for any division leader in baseball. Four of the AL East’s five teams are being outscored. And it could get even bleaker: All five teams have put up these poor results while facing schedules that are significantly weaker than average.There’s plenty of time for things to turn around; four-fifths of the season remain. Even if just one or two AL East teams get better, that could be enough for the division to win another pennant and even the major-league title.Winning pennants and titles has been the forte of the AL East since the majors reorganized after the 1994-95 players’ strike. Teams from the division have represented 36 percent of all AL clubs but have won 11 of 19 pennants and eight of 10 World Series titles for AL teams — twice as many as any NL division. (NL East teams have won four.) The division has also claimed 71 percent of available AL wild-card slots, twice as many as it would be expected to by chance alone.Much of that postseason success was achieved by New York and Boston, and the division has had plenty of underachieving clubs, too. But the AL East’s overall results stand up nicely, too. Its teams won 51 percent of regular-season games between 1995 and 2013. That understates its success because division games each had winners and losers, so they canceled each other out. Against opponents from outside their division, AL East teams won 52 percent of games, including the same proportion against both intraleague rivals and against NL teams.Surprising early-season results often stem from bad luck: Opposing batters’ grounders find gaps in the infield, and their fly balls sail for home runs, at unsustainable rates. One way to measure this luck is to compare teams’ skill-interactive ERA to their ERA. SIERA tries to control for factors beyond pitchers’ control — so it’s roughly what a pitching staff’s ERA would be if it had the same luck, and fielding ability, of the average team. And all five AL East teams have a SIERA below their ERA. The Yankees’ gap is nearly a run per nine innings.Then again, last year, all five AL East teams also had ERAs worse than their SIERAs. And while that doesn’t appear to have been due to bad fielding, this year most of the AL East clubs have below-average fielding.There’s one more sign that its slow start could mean real trouble for the AL’s worst division. A few days ago, Dave Cameron at FanGraphs calculated teams’ expected scoring margins — based on how well they’ve hit the ball, run the bases, fielded and pitched — relative to their actual scoring margins. If the AL East really has been the victim of hard luck, we’d expect its teams to have better scoring margins than they do. Yet overall, they’ve done about as poorly as expected. Because they’ve won even more than we’d expect by scoring margin, if they keep playing as poorly as they have, their luck — and tougher opponents — could make it even harder for AL East teams to maintain the mediocre level they’ve established so far this season.

The team it faces Saturday might be even better than that Bulldog squad. According to Kenpom.com, North Carolina is the No. 11 team in the nation, one spot ahead of Gonzaga. The Tar Heels have handled a few quality teams to this point, beating Stanford 96-72, Michigan 86-71 and No. 21 Tennessee 78-73. Ohio State head coach Chris Holtmann said in watching the film of those wins, it is clear North Carolina is a team that should be making a return trip to the Final Four in 2018.“There’s a reason they’re the No. 5 team in the country,” Holtmann said Thursday. “We’ve watched them play both home and away, they were up on Michigan by 30 with 10 minutes to go. They just can be really, really explosive. Obviously they have a terrific win at Tennessee. Every time you’re watching them play, you’re trying to evaluate things.”But while North Carolina has several big wins on its resume, it also has several standout losses. In its first real test of the season, it was beat down by now-No. 2 Michigan State 63-45, and lost to Wofford 79-75 Wednesday night. The Tar Heels tend to be one of the more dominant teams in the nation across the board, ranking as both a top-20 team by KenPom in both adjusted offensive efficiency (No. 14) and adjusted defensive efficiency (No. 19). But there is one area that could bode well for the Buckeyes. When Ohio State struggles, it is because it gets into turnover trouble. According to KenPom, Ohio State ranks No. 173 in offensive turnover rate. But the Tar Heels have not been a team to force many turnovers this season, ranking only 267th in defensive turnover rate. In North Carolina’s loss to Wofford, it forced just 10 turnovers and scored eight points off those turnovers. But that is one of the only matchups that looks favorable for the Buckeyes. The Tar Heels typically run a zone defense, a scheme that has caused Ohio State fits offensively all season.The Buckeyes also have not dealt with up-tempo teams well. The three teams that have beaten Ohio State — Gonzaga, Butler and Clemson — average just 16.8 seconds per offensive possession this season. The three Power Five teams Ohio State has beaten — Wisconsin, Michigan and Stanford — average 18.2 seconds per offensive possession. The Tar Heels are the seventh-fastest team in the nation on offense, taking just 14.4 seconds on every offensive possession. Ohio State forward Jae’Sean Tate said the team has spent extensive time at practice this week working on transition defense in anticipation for North Carolina’s faster offense.“The way they get the ball up the court so fast even on made-baskets, going to be key trying to make them play in the half-court is basically what we’re going to have to try to do,” Tate said.How will UNC rebound from its loss to Wofford?Both Ohio State and North Carolina wrapped up relatively easy nonconference stretches of their schedules before the upcoming trip to New Orleans. But unlike Ohio State, North Carolina could not handle its business against Wofford, losing 79-75.Wofford shot 43.8 percent from the field and 31.8 percent from beyond the arc overall, but was exceptional in the second half. The Terriers shot 50 percent (17-for-34) from the field, while also maintaining a 40 percent 3-point success rate with 6-of-15 makes. The Tar Heels, by comparison, shot only 36.4 percent from the field and 28 percent from 3-point range during the game. North Carolina, not known to be a 3-point shooting team, fell behind to Wofford and tried to make it all back up in bulk, but went just 5-for-16 from beyond the arc in the second half.The loss to a 25-point underdog could do one or two things to a team: it could serve as sufficient motivation to play as hard as possible the next time out or it could drag down morale. Holtmann and the Buckeyes are preparing for the former, reflecting on how the Tar Heels responded to its first loss of the season to Michigan State.“After they came back from getting beat by Michigan State, they were phenomenal against Michigan and it was incredible. Like I said, up 30 midway through the second half. I think any time you have a veteran team, that’s typically what happens,” Holtmann said. “You look at it, you say, ‘Absolutely, I’m pretty confident that we’re going to get their very best.’”Prediction:UNC wins 76-68
